We are looking for motivated Physical Therapists to support our patients and contribute to exceptional home health outcomes!
Position Summary Physical Therapists provide direct patient care through evaluation and treatment of physical impairments, develop and implement individualized therapy plans, deliver restorative and rehabilitative services, and assist with patient education to improve functional mobility and quality of life in the home setting.
Key Responsibilities
- Provide direct skilled physical therapy services (e.g., therapeutic exercises, gait training, balance training, manual therapy) per established plan of care
- Perform comprehensive patient evaluations and develop individualized treatment plans
- Document assessments, interventions, progress, and patient responses accurately and timely
- Communicate and report changes in patient condition to supervising physician or care team
- Educate patients and families on therapeutic exercises, mobility, safety, and fall prevention
- Assist with coordination of interdisciplinary services and participate in team meetings
- Maintain compliance with agency policies, CDPH, Medicare, and professional practice standards
- Travel to patient homes within the service area
- Qualifications
Current, active Physical Therapist (PT) license in the State of California
- Minimum 1 year of recent physical therapy experience (home health, acute care, outpatient, or rehabilitation preferred)
- Knowledge of home health therapy practices and Medicare regulations
- Strong clinical skills, attention to detail, and communication abilities
- Valid California driver's license, reliable vehicle, auto insurance, and willingness to travel
- CPR certification (BLS) required
- Home health experience and bilingual skills a plus
Job Types: Part-time, Per diem
Pay: $80.00 - $105.00 per hour
