About the Role
Medical Provider will provide comprehensive primary care to patients in the comfort of their homes throughout New York City.
Our patient population primarily consists of elderly, homebound, and medically complex individuals living with multiple chronic conditions who benefit from receiving personalized care where they are most comfortable. This role offers the unique opportunity to build meaningful, long-term relationships with patients and caregivers while delivering comprehensive, patient-centered medicine outside the traditional office setting.
Supported by an experienced Care Coordination and Operations team, you'll focus on providing exceptional clinical care while collaborating with interdisciplinary professionals to improve health outcomes, reduce avoidable hospitalizations, and enhance patients' quality of life.
Primary Responsibilities
- Perform comprehensive history and physical examinations during home visits.
- Diagnose and treat acute and chronic medical conditions.
- Develop individualized care plans.
- Order and interpret laboratory testing and diagnostic imaging.
- Prescribe medications and monitor treatment effectiveness.
- Perform medication reconciliation.
- Coordinate referrals with specialists and ancillary services.
- Educate patients and caregivers regarding disease management and preventive care.
- Collaborate closely with our Care Coordination team to ensure continuity of care.
- Document encounters accurately within eClinicalWorks (eCW).
- Participate in quality improvement initiatives and clinical best practices.

Qualifications
Required
- Active New York State MD NP or PA license
- DEA registration
- Current malpractice coverage
- Eligibility to work in the United States
- Experience in Primary and/or Pediatric Care
Preferred
- Home-Based Primary Care
- Value-Based Care experience
- eClinicalWorks (eCW)
- Bilingual (English/Spanish)

Base Pay: $3,000.00 - $10,000.00 per month
