WHO YOU CAN BECOME: The Licensed Practical Nurse (LPN) is a 35-hour, non-exempt, hourly position who is responsible for providing basic health care needs for all students of LaSalle School, ages 12 to 21. Under supervision of the Registered Nurse (RN) on duty and in collaboration with other team members, the LPN provides direct and indirect patient care in accordance with NY Health Laws Act, professional standards of care, State regulations, and LaSalle philosophy, policies, and procedures. The LPN Also obtains consultation with other ancillary providers, internal and external, on an ongoing basis.
Looking for Mid Shift Medication LPN (10:00 AM – 6:00 PM) and Evening Shift LPN (4 days at 11:00 AM – 9:00 PM or 5 days at 12:00 PM – 8:00 PM).
WHAT YOU’LL DO:
- Review prior day discharge paperwork and provider visit documentation.
- Receive report from on-duty nursing staff and assess shift priorities.
- Assist with student assessments, evaluations, and clinical support needs.
- Identify and process new medication orders from provider appointments.
- Prepare, pack, and administer dinner and evening/night medications.
- Ensure all medications are ordered, documented in the MAR, and accurately maintained.
- Upload and maintain all medication and clinical documentation in the EHR (AWARDS).
- Stock and maintain triage room and medication station supplies.
- Communicate medication changes, appointment outcomes, and provider recommendations to treatment teams and medical providers.
- Monitor and maintain inventory of medical supplies and request orders as needed
- Coordinate follow-up appointments with transportation staff.
- Perform daily medication refrigerator temperature log checks.
- Monitor and update medication refill needs, including blister packs and pro re nata medications.
- Conduct Monday pediatric prep tasks including vitals, height, weight, and room readiness.
- Review stimulant documentation and collaborate with RN staff to ensure accuracy.
- Complete monthly checks of Automated External Defibrillators, first aid kits, and expired supplies.
- Document medication refusals and verify medication accountability.
- Support staff with nursing concerns, urgent needs, and clinical follow-up.
- Maintain and update refill tracking lists and ensure continuity of medication supply.
- Support monthly MAR reconciliation and medication consent verification.
- Assist with home visit medication packing preparation on designated days.
- Maintain organization and cleanliness of all nursing and medication areas.
- Provide clinical support, assessments, and general nursing assistance as needed.
- Provide end-of-shift communication and handoff to incoming staff.
- Other duties, as assigned.
WHAT YOU NEED:
- Active LPN license in good standing.
- Current CPR certification.
