Overview
The Fall Prevention Coordinator will support all activities related to the prevention of falls in the home. Meals on Wheels, a health and wellness provider for older adults in Solano County, is seeking a Fall Prevention Coordinator to oversee the in-home Fall Prevention Program
Duties
- Assess and triage fall risks in-home.
- Coordinate services with independent contractors.
- Order, organize and manage inventory of fall prevention equipment.
- Enter and manage client data and documents for reporting.
- Prepare time sensitive data reports for contract reporting, presentations and grants.
- Participate in outreach activities and community presentations.
- Other duties as assigned.
Qualifications
Must be proficient in Microsoft 365, including word and excel.
Must be very organized.
Excellent oral and written communication and analytical skills.
Ability to enter data quickly and efficiently.
Ability to work well with diverse populations and display a compassionate nature.
Approximately 24 hours per week, T-F, in-office.
Reliable transportation and proof of automobile insurance required.
Must be able to pass background check and drug screen.
This is a part-time non-exempt position located in Fairfield, California.
