Responsibilities include:
- Assess nature of patients' situations by interviewing and reviewing personal history.
- Develop and execute individual treatment plans/care plans.
- Determine appropriate milestones to gauge patient progress.
- Communicate patient updates to all relevant parties.
- Facilitate referrals to other healthcare professionals and programs as needed.
- Maintain accurate documentation.
Qualifications:
Previous experience in social work, with a focus on short term and long-term care patients
Compassionate and caring demeanor
Ability to build rapport with staff, patients, and their loved ones
Strong leadership qualities
Excellent written and verbal communication skills
