Job Summary:
LPC/LPCA will assist clients with accessing resources, coordinating services, and advocating for the needs of individuals or families. This role involves assessing client needs, developing personalized care plans, monitoring progress, and ensuring clients receive the appropriate support and services.

Job Duties:
○ Conduct assessments to determine clients’ physical, emotional, social, and environmental needs.
○ Identify the resources and services required for client support.
○ Develop individualized care plans that outline goals and the steps needed to achieve treatment.
○ Collaborate with clients and with the agency to establish realistic and measurable objectives.
○ Facilitate communication between clients, family members, healthcare providers, and other relevant professionals to ensure effective service delivery within a 48 hour service window.
○ Coordinate services such as hospitalization, housing, employment assistance, mental health services, and skills training.
○ Advocate on behalf of clients to access necessary resources and services.
○ Provide emotional support and guidance to clients and their families in accordance with governing bodies.
○ Track client progress and adjust care plans as necessary.
○ Ensure that clients are receiving timely and appropriate services by uploading all signature documents SARS within 48 hours.
○ Provide regular follow-ups to evaluate the effectiveness of the care plan to make adjustments as needed.
○ Maintain case coordination logs for accurate, up-to-date records of client interactions, progress, and services provided.
○ Prepare reports on client status, treatment plans, and outcomes for review by management.
○ Stay abreast of all governing body rules and guidelines as the relate to the case management position to ensure marketing and communication of agency roles are accurate and compliant.
○ Work with multidisciplinary teams (doctors, social workers, counselors, etc.) to ensure comprehensive care for clients.
○ Participate in case conferences and team meetings to discuss client cases and strategies for improvement.
○ Provide immediate assistance in crises and connect clients with emergency resources if needed.
· Travel Requirements: Depending on the position, travel within the community may be required.
Qualifications:
● Education:
Master’s degree in social work, Nursing, Psychology, or a related field.
● Experience:
○ LPC or LPC-A
● Skills:
○ Medication training and support services.
○ Psycho-social rehabilitative services
○ Skills training for day programs for acute care needs.
○ Strong interpersonal and communication skills.
○ Ability to assess client needs and develop actionable care plans.
○ Organizational skills and attention to detail.
○ Crisis management skills and ability to work under pressure.
○ Knowledge of community resources and support systems.
○ Ability to maintain confidentiality and adhere to ethical standards.
· Work Environment: LPC/LPCA may work in various settings, including hospitals, clinics, social service agencies, or community organizations. Some may also work in clients' homes or in field settings.
Pay: $35.00 - $40.00 per hour
