Job Summary:
Vivo Healthcare Normandy is seeking a highly skilled and dedicated MDS (Minimum Data Set) Coordinator to join our team in Jacksonville, Florida. As a key member of our healthcare organization, the MDS Coordinator will be responsible for ensuring accurate and timely data collection, reporting, and submission to various healthcare organizations and regulatory bodies. Responsibilities:
Collect, analyze, and submit accurate and complete MDS data for residents in accordance with federal and state regulations
Collaborate with interdisciplinary teams to ensure comprehensive care planning and accurate data collection
Identify and resolve data discrepancies and discrepancies in a timely manner
Develop and maintain strong relationships with healthcare providers, caregivers, and family members to obtain accurate information
Participate in quality improvement initiatives to improve MDS data accuracy and completeness
Stay up-to-date with changing regulations and guidelines related to MDS data collection and submission
Requirements:
High School Diploma or Equivalent required; Associate's or Bachelor's Degree in Healthcare, Nursing, or related field preferred
1-2 years of experience in MDS data collection or a related field
Strong analytical and problem-solving skills
Excellent communication and interpersonal skills
Ability to work effectively in a fast-paced environment with frequent deadlines
Proficiency in electronic health records and data management systems
