Job Overview
We are seeking a dedicated and skilled *Physical Therapist* to join our healthcare team. The ideal candidate will provide comprehensive patient care across various settings, including inpatient, outpatient, hospital, home health, and community care. This role requires a strong foundation in physiotherapy principles, anatomy, physiology, and medical terminology to develop and implement effective care plans for diverse patient populations, including pediatrics, geriatrics, orthopedics, and those recovering from acute conditions. The Physical Therapist will play a vital role in discharge planning and collaborating with multidisciplinary teams to promote optimal recovery and mobility.
Duties
- Assess patients’ physical conditions through detailed evaluations of mobility, strength, and functional abilities.
- Develop personalized *care plans* tailored to individual patient needs, incorporating evidence-based practices such as *trigger point therapy* and other physiotherapy techniques.
- Implement therapeutic interventions to improve movement, reduce pain, and restore function across various settings including *acute care*, *home & community care*, *outpatient clinics*, and *hospital environments*.
- Monitor patient progress regularly and adjust treatment strategies accordingly to ensure effective recovery outcomes.
- Educate patients and their families on exercises, injury prevention, and health maintenance strategies.
- Coordinate with healthcare teams on *discharge planning* to facilitate seamless transitions from hospital or clinic to home or community-based care.
- Document patient progress accurately using *EMR systems* while adhering to healthcare regulations and standards.
- Stay current with advancements in *physiotherapy*, *orthopedics*, *geriatrics*, and other relevant fields to provide the highest quality of care.
Skills
- Strong knowledge of *anatomy*, *physiology*, and *medical terminology* relevant to physical therapy practice.
- Experience with diverse patient populations including *pediatrics*, *geriatrics*, orthopedic cases, and those requiring *home & community care*.
- Proficiency in developing and managing comprehensive treatment plans that include discharge planning and specialized therapies such as trigger point therapy.
- Familiarity with various clinical settings such as *inpatient*, *outpatient*, *hospital*, *home health*, and clinics.
- Ability to utilize *EMR systems* efficiently for documentation and patient management.
- Excellent communication skills for patient education and interdisciplinary collaboration.
- Knowledge of treatment techniques specific to areas like *orthopedics* and managing patients with complex needs such as post-surgical recovery or chronic conditions.
