Job Summary:
Modoc Medical Center is seeking an experienced LVN/LPN to provide individualized nursing care to residents within the Skilled Nursing Facility. The LVN will work closely with the Director of Nursing, physicians, RNs, CNAs, residents, families, and other healthcare team members to ensure safe and high-quality patient care.
Requirements:
- Active California LVN license
- Current BLS/CPR certification
- Strong patient assessment and clinical skills
- Excellent communication, organizational, and time-management skills
- Ability to work effectively in emergency situations and as part of an interdisciplinary team
- Dependable with a strong attendance history
- Must wear Caribbean Blue scrubs while on duty
Key Responsibilities:
- Perform patient assessments and report abnormal findings
- Administer medications and treatments as ordered
- Assist with IV starts, blood draws, EKGs, wound care, and other nursing procedures
- Develop and follow individualized patient care plans
- Document patient care accurately and timely
- Respond appropriately to emergency situations
- Provide patient and family education
- Follow infection-control and patient-safety standards
- Coordinate care with RNs, CNAs, physicians, and other departments
Schedule: Work hours are based on departmental needs and may require flexibility, including working beyond normal hours when necessary.
Pay: $2,250.00 per week
