Job Summary
We are seeking a compassionate and organized Behavioral Health Personal Care Coordinator to support members with behavioral health, medical, and social service needs. This role focuses on coordinating care, connecting members with community resources, and collaborating with providers to improve health outcomes and ensure timely access to services.
*This is a full time, temoporary role with an expected duration of 6 months*
Responsibilities
- Coordinate care for members with behavioral health and medical needs.
- Assist members in navigating healthcare services and community resources.
- Conduct member outreach and respond to referrals and care management inquiries.
- Collaborate with providers and interdisciplinary care teams to support individualized care plans.
- Coordinate transitions of care and assist with authorizations and documentation.
- Maintain accurate case records and ensure compliance with regulatory requirements.
- Participate in quality improvement initiatives and departmental meetings.
Qualifications
- Bachelor's degree in a healthcare-related field or equivalent experience.
- Three or more years of experience in care coordination, case management, behavioral health, or a related healthcare setting.
- Knowledge of behavioral health, case management, and community resources preferred.
- Strong communication, organizational, and problem-solving skills.
- Experience with electronic medical records and Microsoft Office.
