Responsibilities
- Conduct thorough patient assessments to evaluate psychological, social, and emotional needs using clinical interview techniques and diagnostic tools.
- Develop and implement tailored care plans that align with clinical ethical guidelines and social work practice standards.
- Coordinate community resources and support services to assist patients with housing, financial assistance, transportation, and other social determinants of health.
- Document all interactions accurately while maintaining compliance with HIPAA regulations.
- Collaborate closely with interdisciplinary teams—including medical professionals, nurses, therapists, and case managers—to ensure holistic patient care.
Requirements
- Valid state licensure as a Certified Social Worker (CSW).
- Proven experience in social work practice within healthcare settings.
- Strong knowledge of medical terminology, ICD-10 coding standards, and Medicare/Medicaid policies.
- Demonstrated expertise in counseling, behavioral health interventions, and working with diverse cultural backgrounds.
- Ability to perform patient assessments accurately and develop appropriate treatment plans aligned with clinical ethical guidelines.
- Excellent communication skills for client interaction and interprofessional collaboration.
- Certifications in CPR/BLS (Basic Life Support) are preferred.
- Experience working with geriatric patients, individuals with developmental disabilities or autism spectrum disorder (ASD), dementia care or Alzheimer’s care is highly desirable.
