What You'll Do
- Perform comprehensive assessments and develop individualized care plans
- Manage ongoing visits and provide patient and family education
- Complete OASIS assessments and clinical documentation
- Coordinate care with physicians and interdisciplinary teams
- Help patients remain safe, independent, and healthy at home
What You BringRequired
- Active Minnesota RN license
- Three or more years of nursing experience
- Strong assessment and communication skills
- Reliable transportation and valid driver's license
Preferred
- Home health experience
- OASIS experience
- Previous case management experience
