The Hockomock Area YMCA is seeking a Pathways Forward Program Manager to oversee the day-to-day coordination of the Pathways Forward Program and scholarship process within the YMCA's Social Action Services (SAS) Center.
This role serves as a primary point of contact for individuals and families seeking financial assistance and access to YMCA programs and services. The Program Manager will guide participants through the scholarship and Pathways Forward process, support enrollment in programs and workshops, monitor participation, and connect individuals with YMCA and community resources that promote overall well-being.
The Pathways Forward Program Manager plays a critical role in creating a welcoming, supportive, and inclusive experience for participants while ensuring applications, referrals, intake information, and program outcomes are accurately documented and tracked.

Essential Functions
- Serve as the primary point of contact for individuals participating in the Pathways Forward and scholarship programs.
- Process scholarship applications and guide participants through enrollment and onboarding procedures.
- Conduct intake conversations to identify participant interests, goals, needs, and appropriate YMCA programs and supports.
- Assist participants with enrollment in YMCA classes, workshops, chronic disease prevention programs, and community-based services.
- Maintain accurate participant records, case notes, and documentation within Casebook and other designated systems.
- Track program participation, enrollments, referrals, outcomes, and follow-up communications.
- Provide a welcoming, organized, and supportive experience for all participants.
- Collaborate with YMCA departments, including Scholarship, Food Access, Childcare, Chronic Disease Prevention Programs, and Social Action Services, to support participant success.
- Connect participants with appropriate internal YMCA programs and external community resources as needed.
- Consult with the Social Worker or leadership team when participant needs extend beyond the scope of the role.
- Support grant compliance activities, including data collection, reporting, and program documentation.
- Participate in team meetings and contribute to ongoing program evaluation and continuous improvement efforts.
- Assist with outreach, participant communications, recruitment, and engagement activities to increase awareness and participation in Pathways Forward programs.
