Schedule: Flexible / Variable – Must be available during standard business hours and after-hours during inspection periods
The Quality Assurance (QA) Officer & Audit Monitor is responsible for ensuring that all Arizona Dream Team I, LLC programs—including Residential Group Homes (all 14 homes and any additional locations), Day Treatment for Adults (DTA), Day Treatment & Training (DTT/DTS), and Group Supported Employment (GSE) programs—operate in full compliance with the Division of Developmental Disabilities (DDD) standards, Arizona state laws, and agency policies and procedures.
This position will perform ongoing internal audits, quality monitoring, and compliance inspections across all operational departments, including Residential, DTA/DTT/DTS/GSE programs, Human Resources, and Care Coordination. The QA Officer will oversee corrective action follow-up, prepare detailed audit reports, and act as a liaison during DDD monitoring visits, ensuring all documentation, records, and staff are prepared and compliant.
Essential Duties and Responsibilities
Conduct internal audits for all group homes, DTA/DTT/DTS, and GSE facilities to ensure compliance with DDD standards, licensing requirements, and internal policies.
Perform audits and compliance reviews of internal departments including Human Resources, Care Coordination, and Administrative functions.
Maintain working knowledge of Arizona Administrative Code (AAC), DDD Provider Manual, Qualified Vendor Agreement (QVA), and other applicable state and federal regulations.
Monitor and evaluate program operations, member records, staff training files, and service documentation for accuracy, completeness, and timeliness.
Develop and implement internal quality assurance tools, audit checklists, and tracking systems aligned with DDD monitoring expectations.
Prepare detailed audit reports summarizing findings, corrective actions, and follow-up timelines; submit to Executive Director and relevant department heads.
Conduct follow-up reviews to verify that corrective actions have been implemented and sustained.
Maintain confidentiality and integrity of all member and staff information reviewed during audits.
Stay current on DDD updates, policy revisions, and audit requirements; attend trainings and workshops as needed.
Coordinate and be physically present during DDD on-site monitoring visits and inspections across all facilities.
Be available after hours or on weekends during inspection periods to support facilities, staff, and management.
Collaborate with Program Managers, Residential Coordinators, and other departments to ensure consistent application of policies and corrective measures.
Provide ongoing feedback and technical assistance to teams to prevent reoccurrence of deficiencies.
Assist in developing internal quality improvement plans and recommend systemic improvements.
Use agency-provided systems and computers to track audits, findings, and trends; maintain organized electronic and physical documentation.
Ensure documentation, postings, and records (e.g., emergency drills, maintenance logs, staff training, incident reports) are accurate and compliant at all times.
Interact appropriately with DDD members during visits and inspections; demonstrate professionalism, empathy, and respect.
Manage personal schedule to ensure timely completion of all assigned audits and inspections across all programs.
Travel to all assigned locations using personal vehicle; maintain a valid driver’s license, insurance, and submit mileage for reimbursement in accordance with agency policy.
Perform additional duties as assigned by the Executive Director to support agency-wide compliance and quality initiatives.
Required Knowledge, Skills, and Abilities
In-depth understanding of DDD standards, Arizona Administrative Code (R6-6), and Qualified Vendor Agreement requirements.
Strong analytical, organizational, and problem-solving skills.
Excellent attention to detail and ability to recognize compliance risks or inconsistencies.
Strong written and verbal communication skills, with the ability to prepare clear and comprehensive reports.
High level of professionalism and integrity.
Ability to interact effectively with staff, management, members, and regulatory agencies.
Proficiency in Microsoft Office Suite (Word, Excel, Outlook) and ability to learn agency-specific software systems.
Ability to work independently and manage multiple priorities under limited supervision.
Comfortable entering group homes and day programs, interacting with members, and observing care and services provided.
Ability to remain calm and professional during audits, inspections, and stressful situations.
Must be detail-oriented with strong follow-up and follow-through practices.
Training and Onboarding
Initial training period: Several months of shadowing and direct instruction in DDD compliance, documentation review, internal auditing processes, and monitoring preparation.
Will transition to an independent, stand-alone role once training is completed and competency is demonstrated.
Ongoing professional development to remain current on all regulatory and agency updates.
Physical and Work Requirements
Regular travel to all assigned homes and facilities.
Ability to walk, stand, and move throughout residential and program facilities.
Ability to lift up to 25 lbs. occasionally for inspection purposes.
Primarily office-based work with field visits across multiple locations.
Compensation and Reimbursement
Mileage reimbursement for travel between agency sites.
Compensation commensurate with experience and education.

Supervision
Reports directly to the Executive Director. Works collaboratively with Program Managers, Residential Coordinators, Human Resources, and Administrative Leadership.
Performance Evaluation
Performance will be evaluated annually (or as needed) based on audit accuracy, timeliness, regulatory compliance, follow-up completion, and ability to maintain agency readiness for DDD monitoring and licensing reviews.
Minimum Qualifications
High School Diploma or GED required; Bachelor’s degree in Human Services, Business Administration, or related field preferred.
Minimum of 3 years of experience in the DDD field, group home operations, compliance, or quality assurance.
Familiarity with DDD Provider Manual, Licensing Standards, and Behavioral Health documentation requirements.
Valid Arizona Driver’s License and reliable vehicle with proof of insurance.
Ability to travel across all agency sites as needed.
Must have a VALID, Level One Fingerprint Clearance Card background check.
Must be able to work flexible hours, including after-hours and weekends during inspection or audit periods.
