Registered Nurse | PRN
Crystal Home Health Care is seeking Full Time or PRN visiting Home Health Registered Nurse for visiting patients in the Chicagoland area. We are looking in the areas of:
- Palos Park area
- Orland Park area
- Country Club Hills area
- Midway Airport Area - Back of the Yards, Garfield Ridge
Summary:
The Registered Nurse is responsible for the delivery of quality patient care services. Responsible for establishing a Plan of Treatment individualized for each patient and supervise the staff delivering the care. You will build relationships with patients and families as you visit them within the comfort of their own homes.
Responsibilities:
- Conduct Comprehensive Assessments (using the OASIS tool), develop and implement Plan of Treatment
- Collaborate with physicians, agency staff and contract services to coordinate patient care
- Initiate appropriate referrals for other medical services when indicated
- Perform various types of skilled nursing visits
- Promote and maintain patient health through teaching and learning appropriate self-care techniques
- Document direct patient services in a timely manner in electronic medical records
- Collaborate with management team and office staff to facilitate timely office processing of paperwork and patient information
- Participate in agency quality improvement activities
- Maintain the highest level of ethical standards
Qualifications:
- Must be a Registered Nurse (RN) with the Illinois Department of Professional Regulation
- Must have a minimum two years of home health experience
- Psychiatric Nursing experience a major advantage
- Current CPR certification
- Must have and maintain a valid IL driver’s license, automobile insurance coverage
- Must be willing to travel to surrounding areas and patients’ homes
- Ability to work independently with minimal supervision and time management abilities
- Ability to successfully complete required background check
- Excellent communication and reporting skills

- Variable per experience in Home Health
