Registered Nurse (RN) - Long-Term Care
We are seeking a dedicated and compassionate Registered Nurse (RN) to join our team at Fairport Rehab & Nursing Center, a premier provider of senior care services in Fairport, New York. About the Role:
As a Registered Nurse (RN) at Fairport Rehab & Nursing Center, you will:
Provide direct patient care to our long-term care residents, including wound care, medication management, and vital sign monitoring
Collaborate with other members of the healthcare team to develop and implement individualized care plans
Conduct thorough assessments and evaluations to identify changes in patient condition and initiate interventions as needed
Demonstrate excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and colleagues
Responsibilities:
Deliver high-quality, patient-centered care in accordance with established standards and policies
Administer medications, treatments, and medical procedures under the supervision of a physician
Monitor and report any changes in patient condition or concerns to the healthcare team
Participate in quality improvement initiatives to ensure continuous improvement in patient care and outcomes
Maintain accurate and up-to-date patient records and documentation

What We Offer:
Compensation and Schedule:
- Pay Range: $40.00 – $42.00 per hour with pay commensurate with certification and years of post-License experience
- Shift Differentials: +$1.50/hr for weekday B & C shifts and +10% more/hour for all weekend shifts
- Available Shifts: Day (7:00am – 3:00pm), Evening (3:00pm – 11:00pm) Nights (11:00pm-7:00AM)
- Schedule Options: Full-Time (37.5 hours per week), Part-Time (22.5 hours per week)
- Weekend Availability: Every other weekend Requirment
- Sign-On Bonus: $4,000 Sign-On Bonus for Full-Time RN's
