As a CNA at Safe Life HHC, you'll play a crucial role in providing compassionate care to our home health patients. Your responsibilities will include assisting patients with their daily activities, ensuring their comfort, and supporting their overall well-being.
Assist with Activities of Daily Living (ADLs): Provide assistance with bathing, dressing, grooming, and toileting for home health patients.
Mobility Support: Help patients with mobility by assisting them with walking, transferring, and using mobility aids.
Vital Sign Monitoring: Take and record vital signs, including blood pressure, pulse, and temperature.
Medication Reminders: Assist patients with medication reminders and ensure compliance with medication schedules.
Nutrition Assistance: Help with meal preparation, feeding, and monitoring dietary restrictions as prescribed.
Hygiene and Comfort: Maintain patient hygiene and comfort, including changing bed linens and providing oral care.
Observation and Reporting: Observe and report changes in the patient's condition, behavior, or symptoms to the nursing team.
Documentation: Accurately document care provided, vital signs, and any changes in the patient's condition in the EMR system (Kinnser by WellSky).
Communication: Maintain clear and effective communication with the patient, their family, and the healthcare team.
Safety Measures: Ensure a safe environment for the patient by following infection control and safety protocols.
Transportation: Provide transportation to medical appointments or assist with mobility as needed.
Emotional Support: Offer emotional support and companionship to patients, promoting their overall well-being.
Collaboration: Collaborate with the healthcare team, including nurses and therapists, to provide comprehensive care to patients.
Patient Education: Educate patients and their families on self-care and safety measures.
Compassion: Show compassion, kindness, and empathy while providing care to home health patients, who may be dealing with pain or illness
