PRIMARY RESPONSIBLITY:
1. Completes initial intake assessment and registration documentation in electronic health record, as per program policies and procedures.
2. Provides individual, family and group therapy to clients and families according to program requirements and based on individualized treatment plans.
3. Develops appropriate treatment plan and clinical/rehabilitative strategies in a timely manner based on client’s diagnosis, clinical presentation, and medical necessity.
4. Provides and implements client services according to treatment plan goals and objectives.
5. Completes evidenced based assessment tools, as needed.
6. Participates as an active member of the treatment team and assists with the treatment plan process.
7. Completes prior authorization documentation, appeals, or other documents and tasks for utilization review purposes, as required and instructed by Utilization Management Department.
8. Manages crisis situations which includes: performing crisis intervention services, seeking out immediate supervision on clinical impressions, and arranging transportation of clients to the appropriate receiving facility, as applicable and as per program requirements.
9. Coordinates discharge planning with treatment team and completes all program discharge documentation for assigned cases, as applicable per program requirements.
10. Must comply with requirements for EHR documentation based on policies and procedures including updates received via email, trainings, staff meetings, and other written communications.
11. Provides services that respect the varied identities people have based on race, ethnicity, age, sexual orientation, gender and gender identification, religion, spirituality, and mental, emotional and physical differences in abilities.
12. Communicates with supervisor regularly, as well as other Center programs to ensure continuity of care.
13. Provides other related functions pertinent to the program, as requested by supervisors.
14. Complies with program and grant specific requirements, as applicable.
MINIMUM REQUIREMENTS:
1. Master’s degree in behavioral health related field or its equivalent.
2. At least one (1) year of experience providing behavioral health services.
3. Experience with behavior modification programs (applicable when working with minors).
4. Must hold a current State of Florida Intern License in good standing.
5. Proficiency in the use of computers, email and word processing software.
6. Valid Florida Driver’s License, as applicable per program.
7. Fluent in English.
