Liberty is an advanced nursing at home agency serving families and healthcare facilities across Southern Maryland. We specialize in providing a complete continuum of care—ranging from premium private duty non-medical support to complex, high-acuity advanced skilled nursing.
Our operations, staffing machinery, and care workflows are fully established. We are a systems-driven organization designed to deliver a flawless, high-quality experience the moment a client is referred. We have built a high-performance engine; now, we are looking for the elite "hunter" who will fuel it.
Position Overview
We are seeking an experienced, highly connected Referral Development Manager to lead our external market growth in Prince George’s and Charles Counties.
This is an autonomous, field-based role. Your primary mandate is to establish and cultivate deep professional relationships with hospital discharge planners, case managers, social workers, physicians, and community partners to secure a steady stream of patient referrals.
You will not be stuck dealing with internal operational bottlenecks or scheduling headaches. Our operations team handles intake, staffing, and clinical execution flawlessly. Your sole focus is building trust, remaining visible, and opening doors.
What You Will Do (Key Responsibilities)
- Execute Daily Field Outreach: Perform 40+ face-to-face field visits per week targeting local hospitals, inpatient rehabilitation facilities, assisted living communities, and physician networks (such as MedStar Southern Maryland, UM Capital Region Medical Center, and UM Charles Regional).
- Drive Service Line Growth: Educate clinical referral sources on our advanced nursing and complex care capabilities (ventilator, tracheostomy, wound care) to secure skilled nursing transitions.
- Build Private Pay Channels: Establish strategic partnerships with local elder law attorneys, wealth managers, and fiduciary trust officers to secure long-term private duty clients.
- Manage the Relationship Pipeline: Methodically qualify and track local referral sources, moving them from initial contact to active, consistent partners.
- Protect Your Network: Act as the trusted liaison between the community and our internal intake team to ensure patient transitions are seamless and your professional reputation is protected.
Who You Are (Requirements)
- Active Professional Network (Required): You must possess current, active, and verifiable relationships with discharge planners, case managers, and social workers within Prince George's County and/or Charles County.
- Experience: Minimum of 2–3 years of successful healthcare business development, marketing, or liaison experience. Backgrounds in Home Health, Hospice, Inpatient Rehab, or Durable Medical Equipment (DME) sales are highly preferred.
- Clinical Credibility (Plus): An LPN or RN background is a major asset, allowing you to speak peer-to-peer with hospital clinicians about complex cases.
- The "Hunter" Mindset: You are highly resilient, view rejection as a math problem, and genuinely enjoy the hustle of face-to-face relationship building. You do not wait for the phone to ring; you make the pavement work for you.
- Mobility: Must have a reliable vehicle and a valid driver's license for daily travel across the Prince George's and Charles County corridor.
What We Offer (Compensation & Perks)
- Competitive Base Salary: $65,000 – $90,000 (Based on experience and depth of local networks)
- High-Upside Commission Structure: Uncapped, milestone-based bonuses paid per successful advanced nursing admission and per billable private-duty hour generated.
- Vehicle Support: flat monthly car allowance.
- Operational Peace of Mind: A fully built, reliable internal support team that backs up your promises to the community and provides exceptional care to every single referral you secure.
Pay: $65,000.00 - $90,000.00 per year
Application Question(s):
- Which specific hospitals, rehabilitation centers, or senior communities in Prince George's or Charles County do you currently have active, operational relationships with? Please name at least three.
