Job Summary
The Hospice RN Clinical Manager is responsible for overseeing the daily clinical operations of the hospice program to ensure the delivery of high-quality, compassionate, and compliant patient care. This leadership role provides supervision and support to interdisciplinary team members, promotes clinical excellence, ensures regulatory compliance, and collaborates with physicians, administrators, and community partners to achieve organizational goals.
Essential Duties and Responsibilities
- Supervise and mentor hospice registered nurses, licensed practical nurses, hospice aides, social workers, chaplains, and other clinical staff.
- Ensure patients receive individualized, compassionate hospice care consistent with physician orders and hospice regulations.
- Coordinate daily clinical operations, staffing assignments, scheduling, and patient care needs.
- Monitor patient care plans and ensure timely assessments, documentation, and interdisciplinary team collaboration.
- Conduct case conferences and participate in interdisciplinary team meetings.
- Review clinical documentation for accuracy, completeness, and regulatory compliance.
- Ensure compliance with Medicare Conditions of Participation (CoPs), state regulations, accreditation standards, and agency policies.
- Participate in quality assurance and performance improvement (QAPI) initiatives.
- Monitor patient outcomes and implement corrective action plans as needed.
- Provide coaching, performance evaluations, and ongoing education for clinical staff.
- Assist with recruitment, onboarding, orientation, and retention of clinical employees.
- Collaborate with physicians, medical directors, referral sources, and community partners to promote continuity of care.
- Respond to patient, family, and staff concerns promptly and professionally.
- Participate in on-call rotation as required.
- Assist with survey readiness and regulatory inspections.
- Maintain patient confidentiality in accordance with HIPAA regulations.
QualificationsEducation
- Graduate of an accredited school of nursing.
- Bachelor of Science in Nursing (BSN) preferred.
Licensure
- Current Registered Nurse (RN) license in the applicable state.
- Current CPR certification.
- Valid driver's license with reliable transportation and automobile insurance.
Experience
- Minimum of three years of nursing experience.
- At least one year of hospice, home health, or palliative care experience preferred.
- Previous supervisory or leadership experience strongly preferred.
Knowledge, Skills, and Abilities
- Strong clinical assessment and critical thinking skills.
- Comprehensive knowledge of hospice philosophy and end-of-life care.
- Working knowledge of Medicare Hospice Conditions of Participation and applicable state regulations.
- Excellent leadership, coaching, and conflict resolution skills.
- Strong organizational and time management abilities.
- Effective written and verbal communication skills.
- Ability to manage multiple priorities in a fast-paced environment.
- Proficiency with electronic medical records (EMR) systems and Microsoft Office applications.
Physical Requirements
- Ability to travel regularly to patient homes and clinical sites.
- Ability to lift up to 50 pounds with or without reasonable accommodation.
- Ability to stand, walk, bend, reach, and sit for extended periods.
- Ability to respond to emergency situations and provide direct patient care when needed.
Working Conditions
