ABOUT ASCENSION REHAB CARE
Ascension Rehab Care (ARC) is a clinician-founded, therapist-led mobile outpatient rehabilitation company serving aging adults across South Carolina. We deliver physical, occupational, and speech therapy directly in patients' homes and senior living communities — meeting people where they live, not where it's convenient for the system.
We were built by clinicians who got tired of high turnover, fragmented communication, productivity over-quality care, and invisible leadership. If you've ever thought, "there has to be a better way to do this" — we built ARC for you.
THE ROLE
As a PRN Physical Therapist with ARC, you'll deliver mobile outpatient physical therapy to aging adults in their homes, independent living communities, and assisted living communities. You'll work with autonomy, see one patient at a time, and build real clinical relationships — not chase productivity numbers.
WHAT YOU'LL DO
- Perform comprehensive physical therapy evaluations using standardized, validated assessment tools. Develop individualized, function-driven plans of care aligned with patient goals and medical necessity.
- Deliver evidence-based interventions focused on mobility, balance, strength, fall reduction, and functional independence. Modify treatment plans based on patient response and objective outcome data.
- Complete evaluations, daily notes, progress notes, and discharge documentation within required timeframes. Communicate clearly with patients, caregivers, families, and referring providers.
- Travel independently to patient homes and senior living communities across your service region.
WHAT YOU'LL BRING
Required:
- Active South Carolina Physical Therapist license.
- Graduation from an accredited Physical Therapy program (DPT or equivalent).
- Strong clinical reasoning and the ability to work independently in mobile environments.
- Proficiency with electronic health records.
- Reliable transportation and a valid driver's license.
Preferred:
- Experience working with older adults.
- Experience in outpatient, home-based, or community-based settings.
- Familiarity with functional outcome measures (TUG, BERG, 6MWT, etc.).
PHYSICAL DEMANDS
- Ability to stand, walk, bend, and lift up to 50 lbs.
- Ability to perform patient transfers and therapeutic activities safely in variable home environments.
- Ability to travel locally by car throughout the workday.

- Compensation: Revenue-based pay — you earn a percentage of the revenue you generate. The more you work, the more you make. Keep most of what you make.
- Mileage reimbursement at $0.50 per mile
