Total Compensation: $80k-$105k
We're seeking an RN to join a dedicated interdisciplinary team providing healthcare services to adults living in residential settings throughout the community.
In this rewarding role, you'll help coordinate medical care, advocate for the individuals you support, collaborate with Direct Support Professionals (DSPs), and promote the highest level of health, independence, and quality of life.
What You'll Do
- Coordinate and oversee healthcare services for individuals residing in OPWDD-certified residential programs, including Intermediate Care Facilities (ICF) and Individual Residential Alternatives (IRA).
- Complete nursing assessments and develop individualized Plans of Nursing Services (PONS).
- Administer medications and treatments as prescribed while monitoring health status and clinical outcomes.
- Visit assigned residences to evaluate health needs and provide clinical guidance to staff.
- Collaborate with physicians, specialists, families, and interdisciplinary teams to coordinate comprehensive care.
- Monitor documentation, medication administration records, and clinical assessments completed by Direct Support Professionals and AMAP-certified staff.
- Conduct AMAP training and recertification in accordance with OPWDD regulations.
- Assist with appointment coordination, specialty referrals, and follow-up care.
- Participate in interdisciplinary meetings and contribute to person-centered care planning.
Qualifications
- Active New York State Registered Nurse (RN) license.
- Minimum of one year of nursing experience supporting individuals with intellectual and/or developmental disabilities.
- Strong written and verbal communication skills.
- Experience working in OPWDD programs.
- Familiarity with ICF and IRA residential services.
