Job Description:
Schedule: Full Time Days
Hours: Monday - Friday 8am - 5pm
Location: Great Lakes Region and service area.

Maintains clinical record information in accordance with established standards and within established time frames.

Displays flexibility in accepting assignment changes and readily accepting assignment and patient care responsibilities, including participation in weekend coverage and call rotation.

Complies with all department policies, procedures, practices, and standards of care.

Evaluates patients as assigned.
-
Assists the physician and home health team in understanding significant social and emotional factors related to health.
-
Performs home assessments to evaluate potential social needs. Conducts visits to evaluate patient needs and implements a plan of social service assistance.

Counsels patients and family members to develop attitudes and behaviors supportive of maximum health practices.
-
Utilizes community agencies and alternative resources to assist patients and families in resolving unmet health care needs, financial, and environmental difficulties.

Reports pertinent observations to the case manager/supervisor and attending physician. Participates in the team process through cooperative efforts with team members in developing the patient care plan and team conferences.

Demonstrates effective communication skills with all team members, patients, and families. Other duties as assigned.
Knowledge, Skills, and Abilities:

Demonstrated knowledge of medical social worker modalities, treatments and skills. [Required]

Mental health counseling, services to the aged/ill. [Preferred]

Broad base of available private and government resources. [Required]

Problem solving abilities. [Required]

Positive interpersonal communication skills. [Required]
