Summary
As a Home Care Nurse at Health Care of South Florida, you will play a vital role in delivering high-quality nursing care to patients in their homes. This position is essential to our mission of providing compassionate and personalized healthcare services, particularly for those requiring specialized post-transplant and rehabilitation care.
Responsibilities
- Provide skilled nursing care to patients in their homes, ensuring adherence to individualized care plans.
- Administer medications and perform medical procedures such as IV infusion and venipuncture as required.
- Monitor patient progress and report changes in condition to the healthcare team.
- Educate patients and families on health management and post-care instructions.
- Collaborate with interdisciplinary teams to coordinate comprehensive patient care.
- Maintain accurate documentation of patient assessments, interventions, and outcomes.
- Ensure compliance with all health regulations and agency policies.
Qualifications
- Current RN license in Florida with relevant home health experience preferred.
- Strong knowledge of anatomy and medical-surgical nursing practices.
- Proficiency in IV infusion techniques and venipuncture procedures.
- Experience caring for infants or children is a plus but not mandatory.
- Excellent communication skills for effective patient interaction and education.
- Ability to work independently while managing multiple patient cases effectively.
