We are seeking an experienced, results-driven healthcare sales professional to join a growing team supporting the expansion of a nationally recognized Program of All-Inclusive Care for the Elderly (PACE). This field-based role is ideal for self-motivated professionals with experience in post-acute care, senior services, managed care, home health, hospice, or healthcare business development who enjoy building relationships throughout their community.
As an Outreach Specialist, you will cultivate referral partnerships, educate healthcare providers and community organizations on PACE services, and guide prospective participants through the enrollment process. Working closely with the Regional Sales Director, you will be responsible for growing your territory, meeting enrollment goals, and serving as a trusted community resource.
- Develop and execute a strategic sales plan to grow referrals and enrollments within your assigned territory.
- Build and maintain strong relationships with referral sources, including:
- Hospitals and health systems
- Skilled nursing and rehabilitation facilities
- Assisted and independent living communities
- Physician offices and medical groups
- Home health and hospice agencies
- Government agencies
- Community organizations and faith-based groups
- Educate referral partners on the benefits and eligibility requirements of the PACE program.
- Identify new business opportunities through research, networking, cold outreach, and relationship development.
- Conduct in-person and virtual presentations to referral partners and prospective participants.
- Achieve monthly enrollment goals of 6–8 new participants following the onboarding period.
- Generate at least 2 self-sourced leads per day with a minimum 35% conversion rate to qualified referrals.
- Complete a minimum of 25 sales call attempts per week, including at least 10 meaningful contacts that result in actionable next steps.
- Consistently meet or exceed territory performance metrics and census growth objectives.
- Conduct educational presentations and consultations with prospective participants and their families.
- Qualify Medicare, Medicaid, Dual Eligible, VA, and private-pay individuals for PACE services.
- Partner with internal teams to transition qualified leads through the enrollment process.
- Receive and manage complex referrals from internal partners, including call centers and other Outreach Specialists.
- Maintain accurate, real-time documentation of all sales activities within Salesforce CRM.
- Track referrals, meetings, presentations, follow-up activities, and enrollment progress.
- Manage an organized sales pipeline through consistent outreach, account management, and follow-up.
- Provide regular reporting on territory performance and sales metrics.
- Work closely with leadership and cross-functional teams to support enrollment growth.
- Participate in community education initiatives and outreach events.
- Contribute ideas and best practices to improve sales strategies and community engagement.
- Perform additional duties as assigned to support organizational goals.
- Healthcare sales, business development, or account management experience required.
- Experience in post-acute care, senior services, managed care, home health, hospice, PACE, Medicare Advantage, or healthcare provider relations strongly preferred.
- Proven track record of meeting or exceeding sales goals.
- Excellent relationship-building, presentation, and communication skills.
- Strong organizational and time-management abilities.
- Comfortable working independently in a field-based role.
- Experience using Salesforce or other CRM platforms preferred.
- Valid driver's license and reliable transportation required.
