Job description
A full-time position is available for a social worker. The successful candidate will work Monday through Friday. Candidate will participate in discharge planning, resident assessments and development of social work care plans; Interview Resident and families; Maintain an excellent working relationship with other staff and coordinate social services to ensure that daily social services can be performed without interruption; Create and maintain an atmosphere of warmth, personal interest and positive emphasis as well as calm environment throughout the facility.
Qualifications: Long-term care and discharge planning experience is preferred
Job Type: Full-time
Pay: $27.50 - $38.00 per hour
