Conduct all General Inpatient (GIP) visits within contracted hospital and inpatient GIP settings, ensuring compliance with hospice regulations, patient care standards, and organizational policies.

Provide ongoing assessment and management of patients requiring General Inpatient level of care due to uncontrolled symptoms such as pain, dyspnea, agitation, nausea/vomiting, seizures, or other acute symptom management needs.

Perform comprehensive nursing assessments and develop individualized plans of care in collaboration with the hospice interdisciplinary team.

Coordinate and communicate closely with hospital physicians, hospice medical directors, bedside nursing staff, case managers, and ancillary providers regarding patient status and symptom management.

Monitor effectiveness of medication regimens and collaborate with providers to obtain new orders needed to achieve symptom control and patient comfort.

Administer and oversee hospicerelated treatments, medications, and interventions in accordance with physician orders and hospice standards of practice.

Ensure patients continue to meet Medicare and hospice eligibility requirements for General Inpatient (GIP) level of care and collaborate with leadership regarding levelofcare reviews.

Facilitate patient transitions, including transfers from GIP to routine home care, skilled nursing facilities, residential care facilities, or other appropriate levels of care as symptoms stabilize.

Serve as a clinical resource between hospital staff, hospice team members, patients, and families.

Provide emotional support, education, and guidance to patients and families regarding disease progression, symptom management, and endoflife care.

Perform hospice admissions in inpatient facilities and other care settings as needed.

Assist with overflow clinical needs as required, including home visits or other patient care responsibilities.

Provide Explanation of Benefits (EOB) education to patients and families, ensuring clarity regarding hospice services and coverage.

Educate interested patients and families on hospice philosophy, services, and eligibility criteria.

Collaborate with interdisciplinary team members to develop and update individualized care plans.

Participate in IDG (Interdisciplinary Group) meetings and contribute clinical updates and care recommendations.

Complete documentation by the end of each workday and maintain accurate, timely clinical records.

Perform additional duties as directed by the Administrator and/or Director of Patient Care Services.

Ensure accurate and timely completion of all GIP visit notes, assessments, medication reviews, symptom management documentation, referrals, and EOB explanations.

Maintain compliance with Medicare Conditions of Participation (CoPs), hospice regulations, and organizational policies.

Participate in quality assurance, performance improvement, and regulatory survey readiness activities.

Ensure all documentation supports GIP eligibility and levelofcare requirements.

2 years of hospice work experience
