We are currently seeking to fill the Hospice RN Clinical Supervisor position.
- Position to be performed in Fresno and surrounding areas
- Compensation: $110K–$125K salary (negotiable, based on experience, etc.).
- 1 year of Hospice experience required.
Position Summary: The RN Clinical Supervisor is responsible for the clinical supervision, coaching, and productivity oversight of RN Case Managers within the hospice program. This role supports the Director of Patient Care Services in maintaining regulatory compliance, clinical excellence, and operational efficiency.
The RN Clinical Supervisor provides mentorship, education, and real-time clinical guidance to RN staff while ensuring patient care is delivered in accordance with Medicare Conditions of Participation, ACHC accreditation standards, and agency policies.
This position also assists with hospice admissions, field support visits, chart review, staff productivity monitoring, and clinical education, with an emphasis on accurate documentation of patient decline and hospice eligibility.
Qualifications
Education
- Graduate of an accredited school of nursing
- BSN preferred
Licensure
- Current California Registered Nurse license in good standing
- Valid driver's license and auto insurance
Experience
- Minimum two years hospice nursing experience preferred
- Previous leadership or supervisory experience preferred
Skills
- Strong clinical assessment and hospice eligibility knowledge
- Expertise in hospice documentation and charting standards
- Ability to coach and mentor professional nursing staff
- Strong communication and problem-solving skills
- Proficiency with electronic medical record systems
Core Responsibilities
1. Clinical Supervision of RN Case Managers
- Provide direct clinical supervision and mentorship to RN Case Managers.
- Serve as a clinical resource for complex patient care situations including symptom management, end-of-life care, and family communication.
- Provide coaching and real-time guidance through in-person visits, phone, video call, and secure messaging platforms.
- Assist RN staff in clinical decision-making and escalation of concerns to physicians or leadership when necessary.
- Promote adherence to hospice standards of practice and agency clinical policies.
2. Clinical Documentation Oversight
- Conduct routine chart audits to ensure:
- Timely documentation
- Accurate and thorough assessment documentation
- Evidence of disease progression and functional decline
- Compliance with hospice eligibility guidelines
- Alignment with the plan of care
- Provide targeted coaching to nurses on documenting terminal decline, symptom burden, and hospice recertification criteria.
- Monitor chart completion timelines and address late charting trends.
3. Productivity and Workforce Oversight
- Monitor RN productivity metrics including:
- Visit volume
- Chart completion timelines
- Time utilization
- Visit scheduling efficiency
- Review time clock records to identify discrepancies between clocked hours and documented clinical work.
- Address overtime trends and provide coaching on time management and workflow efficiency.
- Support staff in organizing patient schedules to ensure appropriate workload distribution.
4. Admissions Oversight and Field Support
- Perform hospice admissions as needed to support agency census growth and patient access.
- Provide field support visits for complex patient situations or when additional clinical expertise is needed.
- Assist nurses with difficult symptom management cases or family care conferences.
- Support staff during crisis situations requiring clinical leadership.
5. Staff Development and Coaching
- Provide ongoing clinical education to RN staff including:
- Symptom management strategies
- Hospice eligibility and decline documentation
- Communication with families regarding disease progression
- Best practices for hospice visit structure and documentation
- Conduct ride-along visits with nurses to assess skills and provide coaching.
- Identify education needs and recommend training opportunities.
6. Regulatory Compliance and Quality Improvement
- Promote understanding and adherence to ACHC accreditation standards and Medicare Conditions of Participation.
- Participate in quality improvement initiatives and clinical performance monitoring.
- Assist the Director of Patient Care Services in maintaining survey readiness.
- Report trends or clinical concerns affecting patient care quality.
7. Communication and Team Collaboration
- Maintain open communication with the Director of Patient Care Services regarding staff performance, operational concerns, and clinical issues.
- Participate in interdisciplinary team meetings and provide input regarding patient care coordination.
- Support effective communication among hospice staff and leadership.
Additional Responsibilities
- Participate in staff meetings, trainings, and case conferences.
- Assist with orientation and onboarding of new RN staff.
- Provide PRN clinical visits as needed to support patient care operations.
- Perform additional duties as assigned by the Director of Patient Care Services.
