Tuscany Village, a family owned skilled nursing and long term care facility located in Pearland, Texas, is seeking one Social Worker (LCSW or MSW) to join our team of dedicated and caring professionals in the role of Director of Social Services. Duties include (but are not limited to): completion of patient assessments/MDS/Care Plans; psych and behavioral management services; overseeing dental, podiatry, optometry, and audiology services; ensuring compliance with Federal and State guidelines for all Social Service functions and discharge planning
This is the perfect opportunity for a Social Worker who enjoys developing close and warm relationships with their patients and families. The ideal candidate is professional, detail-oriented, personable, and truly enjoys working with a primarily geriatric population. You must be able to work independently, and experience with the geriatric population is preferred. Previous LTC/SNF experience is strongly preferred.
