BrightStar Care is seeking a skilled and compassionate PRN Physical Therapist (PT) to provide high-quality, individualized therapy services to clients in the comfort of their own homes throughout the Upstate of South Carolina, including Greenville, Spartanburg, Anderson, and Pickens counties.
This role is ideal for a Physical Therapist who enjoys the autonomy of home-based care and the opportunity to build meaningful relationships with clients and families while helping individuals improve mobility, function, safety, and independence in their daily lives.
This PRN position offers flexibility and autonomy while providing the opportunity to collaborate with our team to meet the needs of our clients. We are seeking a dependable clinician who can accept client visits as they become available, complete evaluations within 48 hours of assignment, and deliver consistent, high-quality care. Reliable availability is essential to maintaining continuity of care, supporting positive client outcomes, and ensuring timely access to services.

Responsibilities
- Provide skilled physical therapy evaluations and treatments in clients’ homes based on physician orders and individualized care plans
- Assess clients’ functional abilities, including mobility, balance, strength, endurance, range of motion, and safety within the home environment
- Develop and implement personalized treatment plans focused on improving independence, reducing fall risk, managing pain, and enhancing quality of life
- Provide therapeutic exercises, gait training, balance training, transfer training, and mobility interventions appropriate to each client’s needs
- Evaluate home environments and recommend strategies, equipment, and modifications to promote client safety and independence
- Educate clients and caregivers on home exercise programs, mobility techniques, fall prevention, and safe use of assistive devices
- Collaborate with physicians, nurses, caregivers, and other members of the healthcare team to coordinate patient care
- Monitor client progress and adjust treatment plans to support changing needs and goals
- Complete timely and accurate documentation in the electronic medical record (EMR)
- Communicate effectively with clients, families, and the BrightStar Care team to ensure continuity of care
Qualifications
- Current Physical Therapist license in the state of South Carolina
- Graduate from an accredited Physical Therapy program
- Experience providing care in home health, outpatient, rehabilitation, hospital, or community-based settings preferred
- Strong knowledge of evidence-based physical therapy practices, anatomy, and functional movement
- Experience treating diverse clientt populations, including geriatrics, orthopedic conditions, neurological diagnoses, and individuals recovering from illness, injury, or surgery
- Ability to independently manage a home-based caseload while maintaining strong communication with the care team
- Proficiency with electronic medical record (EMR) documentation
- Excellent clinical judgment, organization, and interpersonal skills
- Reliable transportation and willingness to travel throughout the assigned service area

Pay: $60.00 - $70.00 per hour
