We are a local, family owned company seeking a compassionate and dedicated Hospice Nurse to Join our compassionate healthcare team as a Hospice Registered Nurse, where your expertise will make a meaningful difference in patients’ lives during their most vulnerable moments. Responsibilities
Patient Care: Provide comprehensive nursing care to hospice patients, including administering medications, managing symptoms, and offering emotional support.
Care Planning: Collaborate with the interdisciplinary team to create and update individualized care plans based on patient assessments and family input.
Family Support: Educate and support families and caregivers on patient care, disease progression, and end-of-life issues.
Documentation: Maintain accurate and timely documentation of patient care, including assessments, care plans, and interventions.
Coordination: Coordinate with physicians, social workers, chaplains, and other team members to ensure holistic care for patients.
Compliance: Ensure all care provided is in accordance with hospice policies, procedures, and regulatory requirements.
Education: Provide ongoing education and training to patients, families, and caregivers on hospice principles and practices.
Experience
- Hospice: 1 year (Required)
- Kinnser: 1 year (Preferred)
Qualifications
- Education: Associate or Bachelor's degree in Nursing (ADN or BSN) from an accredited nursing program.
- Licensure: Current and unrestricted Registered Nurse (RN) license in the state of Texas.
- Experience: Minimum of 1-2 years of experience in hospice, palliative care, or related fields.
- Skills: Strong clinical skills, excellent communication, and the ability to work collaboratively in a team environment.
- Personal Attributes: Compassionate, empathetic, patient-focused, and emotionally resilient.
Work Environment:
- This position involves working in various settings, including patient homes and assisted living facilities.
- May require travel within the service area and flexibility to work evenings, weekends, and on-call shifts as needed.
