ABOUT THE ROLE
Alleviation Healthcare Services, Inc. is a Medicare-certified, Joint Commission-accredited home health agency based in Sherman Oaks. We're hiring a full-time Field RN to serve as the clinical anchor for our Central Los Angeles territory.
You'll manage the full spectrum of skilled nursing visits in your territory — Start of Care, Recertifications, Resumption of Care, Supervisory Visits, and follow-up visits as needed — and participate in regular case conferences as a core member of our clinical team. You'll work closely with our in-office LVN Care Coordinators and clinical leadership to keep patients on track and documentation tight.
WHAT YOU'LL DO
- Conduct Start of Care (SOC), Recertification, Resumption of Care (ROC), and Supervisory Visits for home health patients in the Central Los Angeles area
- Complete accurate, timely OASIS assessments and visit documentation
- Participate in case conferences and collaborate with LVN Case Managers, therapists, and physicians on the plan of care
- Supervise and support LVN and HHA field staff in your territory
- Communicate condition changes and coordinate physician orders promptly
WHAT WE'RE LOOKING FOR
- Active California RN license (required)
- Home health experience with OASIS proficiency (Medicare-certified agency experience strongly preferred)
- WellSky/Kinnser EMR experience a plus
- Reliable vehicle, valid CA driver's license, and current auto insurance
- Current CPR certification
