Responsibilities:
Provide professional social work services to our residents, including assessments, counseling, and care planning
Develop and implement individualized care plans to address the social, emotional, and psychological needs of our residents
Collaborate with other healthcare professionals to ensure comprehensive care and support
Facilitate family conferences and support groups to promote communication and empowerment
Maintain accurate and timely records of services provided
Participate in quality improvement initiatives to ensure the highest level of care and service
Requirements:
Licensure as a Licensed Social Worker (LSW) in the State of New Jersey
2 years of experience as a Social Worker
Strong communication and interpersonal skills
Commitment to providing exceptional care and service to our residents
Master's degree in Social Work (MSW) from an accredited institution
