Overview:
Friends Life Care is seeking a Care Coordinator to support members in Northern Delaware and Chester County, Pennsylvania. This hybrid role works from a home office and in the field with adults age 60+, primarily supporting members who need in-home care. The Care Coordinator maintains regular contact with assigned members and responsible parties, uses assessments to identify needs and risks, and helps connect members with appropriate supports and services.
Care Coordinators work closely with an interdisciplinary team, contribute specialized expertise, and support internal meetings, committees, and work groups as needed. This role is a strong fit for a licensed or credentialed health and wellness professional who enjoys building relationships, supporting older adults in their homes, and working with a collaborative, mission-driven team.
Role Details
- Service Area: Northern Delaware and Chester County, PA
- Hybrid: Home Office, In Office and Field-based
- Regular local travel
- Rotating on-call coverage
- Compensation Range: $70,000 – $80,000
Key Responsibilities:
- Maintain regular contact with assigned members and responsible parties, providing timely follow-up and support based on the member contact schedule.
- Assess member needs, risks, and eligibility for preventive initiatives, community resources, Medicare-related services, and Friends Life Care plan benefits.
- Develop, monitor, and revise care plans that support member safety, wellness, and independence at home.
- Communicate clearly with members and responsible parties regarding continuing care agreements and available services.
- Provide rotating on-call coverage during evenings, weekends, and holidays with the Care Coordination Department.
- Collaborate with the Care Coordination team and other departments by sharing expertise, consulting on member needs, and contributing to meetings, committees, and work groups.
- Maintain accurate documentation, protect confidentiality, comply with HIPAA requirements, and follow applicable policies, procedures, and standards of practice.

Qualifications:
- Degree in nursing, occupational therapy, social work, or another health and wellness-related discipline required.
- Minimum of 3 years of related health care experience required.
- Experience supporting older adults, caregivers, or families in home-based, community-based, or long-term care settings, preferred.
- Must maintain professional credentials and state licensure as appropriate and required.
- Proficiency with Microsoft programs, data entry, and department-specific technology.
- This role requires regular travel throughout the assigned service area for member visits and related responsibilities.
