Responsibilities/Qualifications:
SCN is hiring a Case Manager to provide case management and discharge planning support to residents while building and sustaining strong working relationships across the local healthcare community. The ideal candidate brings a strong case management foundation paired with a background in home health or hospice sales/business development — someone who knows how to build trust with referral sources, represent SCN well externally, and keep referrals moving smoothly from first contact through admission.

Serve as the primary case manager for an assigned caseload — conducting assessments, building individualized care and discharge plans, and providing ongoing case management support from admission through discharge

Build and maintain strong working relationships with hospital discharge planning teams, physicians, and other community referral sources to keep referrals flowing smoothly

Serve as a trusted, responsive point of contact for referral partners — providing daily updates on pending referrals and following up promptly on new ones

Identify and pursue opportunities to grow and strengthen SCN’s referral network, drawing on relationship-building and sales instincts developed in prior homecare or hospice business development roles

Coordinate care conferences and communication among physicians, outpatient providers, family members, and support persons to build a plan the resident is realistically able to follow

Prepare and present discharge summaries to insurance providers to help secure coverage authorization for continued care

Maintain accurate, timely case documentation of assessments, coordination efforts, and care/discharge plans in compliance with regulatory requirements

Prior experience in homecare or hospice sales/business development strongly preferred — with a track record of building and growing referral relationships

Background in case management and/or medical social work preferred

Strong relationship-building and networking skills; comfortable representing SCN to hospitals, physician groups, and other external partners
