Position Summary
Vanguard Healthcare is seeking a dependable, organized, and detail-oriented Filing/Data Processing Clerk to support our home health administrative and clinical operations.
This position is responsible for maintaining accurate and complete patient records, processing clinical documentation, tracking physician orders and plans of care, entering referral information, assisting with document management, and performing general clerical and telephone duties. Accuracy and timely processing are important because this position directly supports clinical compliance and billing operations.
Key Responsibilities
- File and maintain patient record documentation accurately and on a timely basis.
- Date-stamp and process incoming clinical documentation.
- Receive, organize, and route incoming mail and prepare outgoing mail.
- Maintain tracking logs for physician orders, interim orders, and plans of care, including mailed and returned documents.
- Follow up on outstanding clinical documentation as assigned.
- Maintain adequate inventory of medical and office supplies and notify the appropriate supervisor when supplies need to be reordered.
- Maintain supplies of patient clinical record forms and other documentation used by field clinicians.
- Answer telephone calls promptly and professionally and route calls or messages to the appropriate staff member.
- Maintain telephone/call logs as required.
- Process travel and expense documentation.
- Enter new patient referral information into the computer system accurately and promptly.
- Track verification and documentation for skilled home health services.
- Coordinate and verify clinical documentation from admission through discharge.
- Scan, upload, file, and organize patient documents in the electronic medical record.
- Assist with special projects and other clerical duties as assigned.
Qualifications
- High school diploma or equivalent required.
- Approximately 2 years of data-entry or clerical experience preferred.
- Previous healthcare, medical office, billing, or home health experience strongly preferred.
- Previous computer and electronic medical record experience preferred.
- Home health experience is a plus.
- Axxess experience is highly preferred but not required for the right candidate.
- Ability to type approximately 50 words per minute.
- Strong word-processing and computer skills.
- Comfortable using scanners, fax machines, copiers, phones, and other office equipment.

Starting pay will be based on relevant experience, including:
- Home health experience
- Axxess experience
- Medical records experience
- Healthcare billing/documentation experience
- Data-entry experience
- Knowledge of physician orders, plans of care, and home health documentation workflows
Candidates with strong prior home health and Axxess experience may be considered toward the upper end of the range.
