Job Description:
Matrix Home Care Services is hiring Full-Time Home Health LPNs/LVNs to serve patients throughout the Las Cruces, NM area, including Santa Teresa, Chaparral, and Sunland Park. We currently have openings covering both the north end and south end of our service area.
Compensation: $35.00 per hour/ up to $75,800/yr including $3,000 sign-on bonus
- $1,500 paid at your 6-month anniversary of full-time employment
- $1,500 paid at your 1-year anniversary of full-time employment
- No partial or prorated payments.
This role requires ALL of the following — please do not apply unless you meet every item:
- Active LPN/LVN license in New Mexico or a Compact State
- Minimum 1 year of nursing experience
- Minimum 1 year of home health experience
- Experience working under RN/care plan supervision in a home setting
- Current, valid driver's license and reliable personal vehicle
- Current CPR/BLS certification
- Ability to pass a state and federal background check
- Available for full-time hours only (this is not a part-time or PRN position)
- Willing and able to travel throughout Las Cruces, Santa Teresa, Chaparral, and Sunland Park as assigned
Preferred (not required):
- Kinnser/WellSky EMR experience
- OASIS familiarity (data collection support under RN supervision)
Responsibilities:
- Perform skilled nursing visits in patients' homes across our Las Cruces-area service territory (Las Cruces, Santa Teresa, Chaparral, Sunland Park) under the direction of the supervising RN and care plan
- Administer medications, perform wound care, and carry out other skilled nursing tasks as ordered
- Monitor and report patient status, progress, and concerns to the RN case manager
- Reinforce disease management and care education with patients and caregivers
- Maintain accurate, timely documentation
Not a fit if:
- You do not currently hold an active LPN/LVN license
- You have less than 1 year of nursing or home health experience
- You are seeking part-time, PRN, or remote work
- You are not willing to travel across our full service area (Las Cruces, Santa Teresa, Chaparral, Sunland Park)
- You do not have reliable transportation for daily home visits
