About Solero Behavioral Transitions
Solero Behavioral Health’s Transitions Center in Youngstown, Ohio, provides a safe and structured treatment setting for individuals managing mental health and co-occurring substance use disorders. The program offers comprehensive, integrated care—including individual therapy, group counseling, case management, and life skills training to support long-term recovery and emotional stability. Clients receive compassionate, evidence-based support in a setting designed to promote healing, personal growth, and sustained well-being.
Key Responsibilities

Conduct medical assessments and screenings at admission

Monitor client health and progress throughout treatment

Administer medications and manage pain safely
-
Collaborate with providers and clinical teams on treatment plans
-
Document and maintain accurate medical records
-
Educate clients on medications and health-related topics
-
Provide compassionate emotional support, including individual sessions and group facilitation
Qualifications:
-
Graduate of an accredited nursing program
-
Current, unencumbered RN license in the state of Ohio
-
Must be 21 years or older
-
Experience in mental health and/or addiction preferred
-
