Overview
We are seeking a compassionate, reliable, and skilled Psychiatric Mental Health Nurse Practitioner (PMHNP) to join our team and provide on-site psychiatric care within long-term care and skilled nursing facilities. This position requires an experienced PMHNP who can practice independently and manage a patient panel with minimal supervision. Candidates should have at least 2 years of post-licensure independent practice experience, including responsibility for patient panel management, treatment planning, medication management, and continuity of care. In this role, you will round regularly at assigned facilities, conduct psychiatric evaluations, manage ongoing mental health conditions, and prescribe medications as clinically indicated. Duties
- Perform psychiatric assessments, follow-up visits, and medication management for residents in long-term care and skilled nursing facilities.
- Develop and implement individualized treatment plans.
- Prescribe medications as appropriate and monitor effectiveness and side effects.
- Collaborate closely with facility staff, primary care providers, nursing teams, and families.
- Maintain thorough and timely documentation in accordance with regulatory standards.
- Participate in care plan meetings and interdisciplinary discussions as needed.
- Provide compassionate, patient-centered care with a focus on improving quality of life.
Requirements
- Active and unrestricted PMHNP license to work in Colorado
- National certification as a Psychiatric Mental Health Nurse Practitioner
- At least 2 years of independent PMHNP practice experience.
- DEA registration
- Experience in geriatrics, long-term care, or medically complex populations (preferred but not required)
- Strong clinical judgment and ability to work independently
- Excellent communication and collaboration skills
Pay: $135,000.00 - $145,000.00 per year
