Title - RN: Case Manager
Location - Vidalia, GA
Shift - M-F 8 AM- 5 PM , (on call once a month for one week)
License Requirements: RN GA, compact license
Benchmark Requirement: Minimum of 30 points per week
Mileage reimbursement: 0.43 cent per mile
Counties covered: Johnson, Emanuel, Toombs, Montgomery, Tatnall, Evans, Candler, Jeff Davis, Treutlen
Preferred Skills: HCHB, Home Health, clinical skills ( wound vac, picc line, ostomy care, IV abx, wound care )
Primary Responsibilities: Perform comprehensive assessment of high risk members. Collaborate with primary care providers to ensure the implementation of an individualized, comprehensive care plan with specific interventions designed to engage the member.
