The RN develops, coordinates, and implements the Interdisciplinary plan of care for their assigned patients in various hospice settings to ensure the best possible quality of life. RN Case Manager provides all aspects of care for assigned hospice patients in facilities including symptom management, patient/family education, completes all patient assessments and documentation in a timely manner, orients new staff, and participates in Quality Improvement activities. The RN Case Manager is responsible for meeting regulatory requirements of nurses for patients on Hospice,
Supervisory Responsibilities:
- Supervises the Hospice Aide (HA) every two weeks and initiates Hospice Aide Plan of Care. Supervises Licensed Practical Nurse (LPN), no more than 14 days post LPN visit
Duties/ Responsibilities:
- Performs all duties, tasks, and requirements of case management of hospice patients and families. Oversees all care delivered by the interdisciplinary team members (IDT) for their assigned patients/families. *
o Provides case management services for an assigned number of patients
consistent with BBH’s annual budgeted caseload per FTE.
o Implements, documents, and updates the comprehensive IDT plan of care to reflect patient and family needs. Sets realistic and meaningful patient/family goals and interventions with input from IDT and the attending physician.
o Maintains concise and timely communication with the physician and other IDT members.
o Demonstrates a knowledge of best practice in end-of-life care and support, pain and symptom management.
o Meets patient needs in a compassionate and timely manner.
o Provides direct care as requested throughout Leon County.
- Demonstrates knowledge of clinical practice, case management and performs nursing procedures in accordance with current best practice standards and organizational policies and procedures. *
o Demonstrates a knowledge of best practice in end-of-life care and support, pain and symptom management and the accompanying clinical skills.
- Demonstrates high level of efficiency and accuracy in documenting in an electronic medical record (EMR), this includes all comprehensive and required physical assessments. *
o Documentation reflects required hours spent in direct patient contact and all work is completed within agency guidelines.
o Documentation in the medical record reflects appropriate interventions for optimal and ongoing pain and symptom management.
- Demonstrates competence in all aspects of the Hospice Intake & Admission process. *
- Identifies educational needs and provides appropriate education to patient/family and community partners. *
- Follows all BBH Policy and Procedures and abides by BBH mission and values. *
- Represents Big Bend Hospice in a positive, productive, and professional manner, and demonstrates excellent communication and customer service skills with all patients, family, facility staff, physician’s offices, referral sources and coworkers. *
- Participates in orienting new employees, nursing students, family practice physicians, volunteers and others as requested. *
- Practices fiscally sound judgement as evidenced by appropriate use of supplies, equipment, medication, laboratory, and patient therapies following BBH policies, procedures, and philosophy. *

- Participates in Quality Assurance meetings and other QA (Quality Assurance) activities as assigned. *
