Position Title: Community Outreach and Referral Liaison (Healthcare)
Reports To: Chief Executive Officer / Program Director
Employment Type: Part-Time, 20 hours per week
Work Arrangement: Hybrid — office-based, community-based, and approved remote outreach work
Location: Randallstown, Maryland and surrounding service areas.
Compensation / Expected Salary: $22.00–$24.00 per hour starting pay, based on education, experience, knowledge of Maryland DDA services, referral development experience, and demonstrated ability to build community partnerships and generate qualified client referrals. SHR would offer a performance-based bonus as well.
Position Overview
Summit Healthcare Resources, Inc. (SHR) is seeking a motivated and relationship-focused Community Outreach and Referral Liaison to support referral development, community outreach, and client acquisition for our Maryland DDA-approved services.
This part-time hybrid role is ideal for someone with experience in developmental disabilities services, human services, healthcare outreach, case management, or referral coordination.
The successful candidate will build professional relationships with Coordinators of Community Services (CCSs), families, providers, schools, hospitals, and community organizations while helping SHR increase qualified referrals, tours, and appropriate client placements.
Key Responsibilities
- Build and maintain referral relationships with CCSs, case managers, families, providers, hospitals, schools, and community partners.
- Promote SHR services, openings, capacity, and service areas through calls, emails, meetings, provider events, and community outreach.
- Identify referral opportunities and follow up consistently with potential clients, families, and referral sources.
- Coordinate and support tours, meet-and-greets, intake communication, and referral follow-up.
- Explain SHR services clearly and professionally, including residential services, personal support, supported living, community development services, employment services, respite, transportation, and nursing support.
- Maintain accurate referral tracker records, outreach notes, contact lists, and follow-up dates.
- Submit a weekly outreach report and participate in weekly supervision meetings.
- Represent SHR professionally, maintain confidentiality, and avoid making service commitments without leadership approval.
Performance Expectations
This role is measured by documented outreach activity, relationship-building, timely follow-up, qualified referral leads, scheduled tours or meetings, accurate documentation, and contribution to client acquisition.
SHR offers a performance-based bonus after verified client acquisition outcomes, subject to agency policy and available funding.
Required Qualifications
- High school diploma or equivalent required; associate or bachelor’s degree in human services, social work, healthcare administration, business, marketing, communications, or a related field preferred.
- Experience in healthcare, human services, developmental disabilities services, case management, outreach, or referral coordination preferred.
- Knowledge of Maryland DDA services, waiver services, CCS agencies, and person-centered support is strongly preferred.
- Strong verbal and written communication skills.
- Excellent interpersonal and relationship-building skills.
- Ability to communicate professionally with families, CCSs, providers, community partners, and internal staff.
- Ability to work independently, follow up consistently, and meet outreach goals.
- Basic computer skills, including email, Microsoft Office, Google Workspace, and basic tracking systems.
- Valid driver’s license and reliable transportation preferred.
Preferred Skills and Competencies
- Knowledge of Maryland DDA services, waiver programs, Coordinators of Community Services (CCSs), person-centered practices, and services for individuals with developmental disabilities.
- Experience with residential services, personal support, supported living, community development services, employment services, respite, or intake coordination.
- Strong networking, presentation, customer service, organization, and relationship-management skills.
- Experience generating referrals, coordinating admissions, scheduling tours, or supporting client placement.
Work Arrangement
This is a hybrid role involving office-based work, remote outreach, and community-based travel. Duties may include calls, emails, referral follow-up, documentation, meetings, tours, provider visits, outreach events, and reporting.
Approved business mileage will be reimbursed according to SHR policy. Normal commuting and unapproved travel are not reimbursable.

- Do you have experience working in healthcare, developmental disabilities services, behavioral health, social services, or a related field?
- Have you previously worked with referral sources such as CCSs, case managers, providers, hospitals, schools, community organizations, or advocacy groups?
