Nurse Practitioner / Physician Assistant – In-Home Medicare Care

Position Overview
We are seeking a California-licensed Nurse Practitioner or Physician Assistant for a field-based role supporting Medicare members throughout the Ventura area.
This position is focused on independent in-home member visits, including new-member assessments and ongoing care management. The ideal provider has experience with Medicare, Medicare Advantage, geriatric care, primary care, chronic disease management, home-based care, or health risk assessments.
This is not a clinic-based or telehealth-only position. The provider will travel to members’ homes within an assigned local territory, complete comprehensive assessments, review chronic conditions and medications, identify gaps in care, and document findings accurately. Visits are completed independently in the field, with physician support and care coordination resources available for escalation, care planning, and follow-up.
What You’ll Be Doing
- Conduct independent in-home visits with Medicare members in the Ventura area
- Complete new-member assessments and ongoing care management visits
- Perform focused physical exams, medication reconciliation, and chronic condition reviews
- Identify care gaps, social needs, safety concerns, and conditions requiring follow-up
- Document assessments accurately and timely in the care platform/EMR
- Support accurate diagnosis review, HCC/risk adjustment documentation, and care planning
- Coordinate with physicians, case management, primary care providers, and other clinical resources as needed
- Escalate urgent or acute clinical concerns when appropriate
- Manage a defined local territory and meet expected visit, quality, and documentation standards
What We’re Looking For
- Active, unrestricted California Nurse Practitioner or Physician Assistant license
- National board certification: AANP, ANCC, or NCCPA
- Active DEA registration, or ability to obtain
- Current BLS/CPR certification
- Valid California driver’s license, reliable vehicle, and active auto insurance
- Comfort conducting independent in-home member visits
- Strong documentation skills and comfort using EMR/computer systems
- Ability to work full-time hours in a field-based environment
Strongly Preferred Experience
- Medicare or Medicare Advantage experience
- Geriatric, senior care, complex care, chronic care, or primary care experience
- In-home assessments, annual wellness visits, health risk assessments, or HCC/risk adjustment experience
- Home-based care, home health, PACE, value-based care, or care management experience
- Bilingual Spanish is a plus

Pay: $70.00 - $80.00 per hour
