Essential Duties and Responsibilities
Client Relations
· Serve as the primary point of contact for clients, families, referral sources, and community partners.
· Complete client intakes, assessments, reassessments, and onboarding documentation.
· Develop, review, and update individualized client care plans within the ClearCare system.
· Conduct monthly client visits to assess service quality, client satisfaction, and care plan effectiveness.
· Visit clients in hospitals, rehabilitation facilities, nursing homes, assisted living communities, and other healthcare settings as needed.
· Identify client concerns and coordinate timely resolutions with the Director of Operations and interdisciplinary team.
· Work collaboratively with scheduling and recruiting teams to ensure smooth client transitions and continuity of care.
· Maintain accurate client documentation while ensuring compliance with HIPAA and organizational policies.

Marketing & Business Development
· Develop and execute annual and quarterly marketing strategies aligned with organizational growth objectives.
· Prepare and maintain a rolling four-week marketing calendar.
· Build and maintain relationships with physicians, hospitals, rehabilitation centers, skilled nursing facilities, case managers, social workers, insurance providers, and community organizations.
· Generate new referrals through networking, relationship building, presentations, and community outreach.
· Represent Green Leaves ADHC at networking events, health fairs, community meetings, and professional organizations.
· Coordinate educational seminars, wellness events, and community outreach initiatives.
· Monitor referral trends and develop strategies to increase referral volume and market share.
· Maintain referral and business development activities within Well Sky, ClearCare, or other CRM systems.
· Prepare weekly and monthly reports on marketing performance, referral activity, and business development outcomes.
· Analyze competitor activities and market trends to identify opportunities for organizational growth.

· Coordinate social media platforms to enhance community engagement and brand awareness.
· Develop newsletters, brochures, presentations, promotional materials, and marketing collateral.
