- Graduate of an accredited Physical Therapy program
- Current state license as a Physical Therapist (PT)
- Strong clinical, communication, and documentation skills
- Experience in a skilled nursing or rehab setting preferred (new grads welcome!)
- Passion for providing high-quality patient care
- Evaluate residents and develop individualized treatment plans
- Provide therapeutic interventions to improve mobility, strength, and function
- Collaborate with interdisciplinary team members (nursing, OT, SLP, physicians)
- Monitor and document patient progress accurately and timely
- Educate residents and families on treatment plans and home exercise programs
- Ensure compliance with all regulatory and clinical standards
