These services include a Food Distribution service for the needy, the provision of NDIS Services and now we have been approved as an Aged Home Care Provider. We are building a professional Team to provide a superior care service for the elderly in communities around Australia.
We have an opening for an
Aged Care Coordinator who would oversee and develop the Aged Home Care Service for H&F. The role is full time.
This is an exciting opportunity for someone who believes in the H&F Mission and Vision to have a significant impact in people’s lives and to help empower local churches to be connected and involved in their local communities in an engaging and meaningful way. The role includes
- Working under the direction of the GM and Aged Home Care Committee
- Identifying and collaborating with local churches to provide care for the elderly in their community.
- The induction and training of new staff
- The sign up of new consumers directly or through our sub-contractor arrangements
- Reporting against our Quality Systems and Aged Care Policies to the Aged Home Care Committee.
- Oversight of financial management to ensure superior service to consumers and the viability of the Home Care service.
- Maintenance of the Aged Care Quality Standards within the service
- Implementation of a robust staff education and training program
- Monitoring of services provided
- Undertaking assessments and preparing care plans in collaboration with consumers
- Monitoring staff performance and culture
- Reviewing incidents and complaints
- Reporting of mandatory indicators and SIRS to Commission and Board
- Implementation of internal and external audits across clinical and non-clinical services

Typical Qualifications
- Management experience and leadership skills.
- Experience working in the Aged Care Industry
- Clinical Care qualifications would be beneficial for the applicant. (Bachelor of Nursing or Bachelor of Health Science Nursing or equivalent)
