What You’ll Do
- Advocate for clients in medical appointments and healthcare settings
- Provide clinical guidance to staff (medications, diagnoses, care plans)
- Coordinate care with families, providers, and regional centers
- Create and lead medical trainings for staff teams
- Track health trends and improve care practices
- Participate in interdisciplinary planning meetings
What We’re Looking For
- Active RN license (CA)
- 2+ years nursing experience (hospital, SNF, or similar)
- Strong communicator who enjoys collaboration and teaching
- Comfortable in a hybrid role (office + field)
