Job Description
· Participates in planning, directing and coordinating patient care.
· Observes, documents and reports accurately and promptly.
· Communicates effectively with staff, clients, families and physicians.
· Participates in case conferences and staff meetings.
· Ability to be self-directing and to function without close supervision.
· Follows agency policies, procedures and accepted standards of practice.
· Completes comprehensive reassessments of the patients’ needs at appropriate time points.
· Participates in ongoing education or training as required.
· Participates in the emergency preparedness program.
· Participates in Quality Assessment performance improvement Program.
· Observes HIPAA compliance and the ability to honor patient/family rights and confidentiality.
Job Requirements
Educational and Experience Requirement:
· Physical therapy degree from an accredited program.
· A valid and current state physical therapist license in good standing.
· Minimum of two years of home care experience preferred, or sub-acute, or acute care setting.
Knowledge and skills required:
· Ability to use an electronic health record.
· Current CPR certification.
· Driver’s license and current automobile liability insurance.
· Strong written and verbal communication skills
· Excellent critical thinking and interpersonal skills.
