Job Summary:
Chattahoochee Hospice is a nonprofit community-based hospice agency providing patient care to residents of Chambers, Lee, and Randolph counties in Alabama, and Troup and Harris counties in Georgia. We are seeking a full-time RN with broad-based nursing experience to provide excellent patient care. The RN Case Manager is responsible for planning, organizing and directing hospice care for terminally ill patients in their homes. The Case Manager will complete an initial assessment for the patient and family to determine hospice needs, provide a complete physical assessment history, write and initiate a Plan of Care, and re-evaluate needs of patients and revise the Plan of Care as necessary. The Case Manager will work in cooperation with the family/caregiver and the hospice interdisciplinary team members. Excellent employee benefit package including twenty-six paid days off and ten paid holidays annually for full-time employees. Hours are Monday-Friday 8:30-5:00.
Skills / Requirements:
Current unrestricted Alabama and Georgia State license to practice as a registered nurse. At least one year of professional medical-surgical nursing experience required with one year of hospice or home health care experience preferred.
