Job Overview:

Responsibilities:
- Plan, organize and direct home health care services, from initial evaluation and assessment, pain and medication management, and care plan development.
- Ensure high-quality patient experience and positive health outcomes through a comprehensive and personalized plan of care, proactively making revisions based on patient needs.
- Prepare complete and timely clinical notes, coordinate care delivery with an interdisciplinary team and collaborate with the patient's primary care physician to ensure continuity of care.
- Provide coaching and education to patients and their caregivers based on clinical diagnosis, empowering them to self-manage their condition, prevent complications, and improve quality of life.
- Perform diagnostic tests and analyze results to guide patient care.
- Ensure the safe and efficient use of medical equipment.
- Maintain a clean and safe working environment in compliance with healthcare procedures and regulations.
- Monitor and ensure the proper documentation of the patient’s health status.
- Supervise and train less experienced nurses or healthcare support staff.
Qualifications:
- Graduate of an accredited school of nursing.
- Valid and current Registered Nurse license in the State of California.
- Minimum of one year of experience as a Registered Nurse, preferably in a hospital or clinic setting.
- Strong clinical knowledge and skills.
- Knowledge of electronic health records (EHR) systems.
License/Certification:

- CPR Certification (Required)

Competitive Landscape and Compensation
The compensation range for this position is $65.00 to $80.00, commensurate with experience and qualifications. Care Must also offers a comprehensive benefits package, including health, dental, vision, PTO, paid holidays, and flexible spending accounts.
